Follow the steps below to apply for financial assistance from the Office of the Sangguniang Bayan. Ensure all required documents are complete before visiting the office to avoid delays in processing.
- Visit your Barangay Hall and request a Certificate of Indigence from the Punong Barangay.
- Proceed to the Municipal Social Welfare and Development Office (MSWDO) and request a Certificate of Indigence.
- Secure a copy of your Hospital Bill or Doctor's Prescription indicating the required medicines or laboratory tests.
- Compile all documents and proceed to the Office of the Sangguniang Bayan at the Municipal Building.
- Submit your complete documents to the designated SB staff for review and processing.
- Wait for the assessment and approval of your request by the Sangguniang Bayan members.
- Once approved, the financial assistance will be released through the prescribed process of the office.
